 Job Description Licensed Social Worker/Care Coordinator

Position Summary
The Licensed Social Worker/Care Coordinator will provide professional social work services designed to support the development and well-being of children and their families. Services include but are not limited to:

Social work services includes but is not limited to the following

  1. Making home visits to evaluate a child’s living conditions and patterns of parent-child interaction;
  2. Preparing a social or emotional developmental assessment of the child, with the context of the family;
  3. Providing individual and family-group counseling with parents and other family members, and appropriate social skill-building activities with the child and family;
  4. Working; with those problems in the living situation (home, community, and any center where early intervention services are provided) of the child and the family that affect the child’s maximum utilization of EI services;
  5. Identifying, mobilizing, and coordinating community resources and services to enable the child and the family to receive maximum benefit from EI services;
  6. Conduct an Family Direct Assessments (FDAs) to identify family resources, priorities, concerns, as well as the supports and services necessary to meet the developmental needs of the child;
  7. G Conduct an Multidisciplinary Development Evaluation (MDE) to determine initial Part C eligibility and/or to identify present levels of development. The MDE includes an evaluation of the child’s present level of functioning in each of the following developmental areas: cognition; physical (including vision and hearing); communication; social or emotional; and adaptive, and also include a review of pertinent records related to the child’s current health status and medical history;
  8. Function as a Primary Service Provider (PSP) to serve as the liaison between the family and other team members in order to support all outcomes and objectives in the child’s Individualized Family Support Plan (IFSP) by utilizing coaching practices and receiving support from Consultant’s who represent the identified areas of need in order to support the child’s family to strengthen their abilities in promoting the child’s learning and development with daily routines and activities;
  9. Function as a Consultant by utilizing coaching practices to support the PSP, parents, land other caregivers, to strengthen their abilities and their capacity in promoting the child’s learning and development within daily routines and activities; and
  10. Participate in the IFSP) meetings and other meetings (i.e., Family Support Team (FST) meeting, Joint Visit Planning (JVP) meeting, Transition meeting) regarding the child’s progress.

NOTE 1: Social Workers may also provide Care Coordination services (Refer to Care Coordination services Section).

NOTE 2: Licensed Clinical Social workers may also provide psychosocial assessments, diagnostic impressions, diagnostic evaluations and/or psychotherapy.

Position Qualifications

Social Worker must have a current, active Licensed Social Worker in the State of Hawaii (i.e., LSW, LCSW).

Bachelor’s Degree preferably in a social service, education field or other related fields.

Other Duties May Include:

  • Participating in fundraising activities for UCPAH.
  • Participating with UCPAH staff to ensure safety, sanitation and security of office assets of the organization.
  • All other duties assigned.
