Job Description: Care Coordinator (Early Intervention Services)
Position: Care Coordinator
Location: Honolulu, HI (in-office and field work)
Position Summary:
United Cerebral Palsy Association of Hawaii is seeking a compassionate and dedicated Care Coordinator to provide and coordinate services for children from birth to age 3 with Special needy, developmental delays or disabilities in the Honolulu area as part of Hawaii’s Early Intervention (EI) Services. This position plays a crucial role in assisting families throughout the Early Intervention process, ensuring children and their families have access to appropriate services and resources, and supporting their developmental progress from intake through exit. Each child and family is assigned a Care Coordinator. Care coordination services are provided to assist the family throughout the EI process from intake through exit including linking them with services and resources. Each family must be assigned a Care Coordinator. Care Coordination services includes but is not limited to the following:
- Provide intake services:
- Assisting parent(s) in obtaining access to needed EI services and other services identified in the individualized Family Support Plan (IFSP), including making referrals to providers for needed services and scheduling appointments for the child and their families;
- Coordinating the provision of EI services and other services, such as educational, social and medical services that are not provided for diagnostic or evaluative purposes, that the child needs or is being provided;
- Coordinating evaluations and assessments;
- Conduct Family Direct Assessments (FDAs) to identify family resources, priorities, concerns, as well as the supports and services necessary to meet the development of their child;
- Conduct a Multidisciplinary Development Evaluation (MDE) to determine initial and ongoing Part C eligibility and/or to identify present levels of development. The MDE includes an evaluation of the child’s present level of functioning in each of the following developmental areas: cognition; physical (including vision and hearing); communication; social or emotional; and adaptive, and also includes a review of pertinent records related to the child’s current health status and medical history;
- Facilitating and participating in the development review and evaluation of IFSP.
- Coordinating and facilitating weekly Family Support Team (FST) meetings with the team to discuss child’s progress, answer questions the family/team members and obtain coaching support from team members.
- Conducting referral and other activities to assist families in identifying available Early Intervention Providers;
- Coordinating, facilitating and monitoring the delivery of services, required under this part to ensure that the services are provided in a timely manner;
- Conducting follow-up activities to determine that appropriate EI services are being provided;
- Informing families of their rights and procedural safeguards, as set forth in this part and related resources;
- Advocating for accessibility to all services which could benefit the child and family; and
- Facilitating the development of transition plan to preschool, school,
or, if appropriate, to other services.
Position Qualifications:
Care Coordinator shall minimally have a Bachelor’s degree (preferably in a social service, education field, or other related fields).
Other Duties May Include:
- Participating in fundraising activities for UCPAH.
- Participating with UCPAH staff to ensure safety, sanitation and security of office assets of the organization.
- All other duties assigned.
